The clinical case
This case report describes the nutritional management of a 32-year-old male patient with Crohn’s disease since 2022, who was hospitalized for partial intestinal obstruction and a weight loss of 4 kg over three months. The MUST screening (score of 3) and GLIM criteria confirmed moderate malnutrition, with reduced muscle mass and deficiencies in vitamin B12, folate, vitamin D, and albumin.
In preparation for ileal and colonic resection, a 6-month nutritional regimen (3 months preoperative and 3 months postoperative) was implemented, consisting of a low-residue, lactose-free, semi-liquid, divided-meal diet, supplemented with LH VIOLA (6 scoops/day, 600 kcal and 32.5 g of protein/day).
The results
At the end of the treatment, the patient gained 4 kg, with a BMI increase from 20.5 to 21.7 kg/m² and an improvement in muscle mass. Biochemical parameters normalized: vitamin B12 from 162 to 349 pg/mL (+115%), folate from 2.3 to 6.1 µg/L (+165%), and albumin from 29 to 32 g/L (+10%). Quality of life improved significantly, with a return to daily activities and subsequent discontinuation of ONS.
The clinical message
This case highlights how a personalized nutritional intervention—a low-residue diet and oral supplementation tailored to individual needs—can effectively support patients with stenosing Crohn’s disease during the perioperative period, preventing complications, promoting muscle mass recovery, and improving quality of life.
